About the Role

Join CVS Health's Care Management team as a Remote Care Management Support Coordinator and help connect members with the clinical resources and support they need to manage chronic conditions and complex health challenges. Working alongside licensed nurses, social workers, and care managers, you will provide essential administrative and coordination support that enables our clinical teams to focus on delivering high-quality, patient-centered care. What You'll Do

  • Schedule and coordinate care management appointments between members, providers, and clinical staff
  • Maintain accurate member records in care management platforms and electronic health systems
  • Contact members to gather health information, confirm appointments, and relay care plan updates
  • Process referrals and prior authorizations in accordance with health plan guidelines
  • Support clinical care managers with administrative tasks, data entry, and reporting functions
  • Identify and escalate member concerns or urgent health issues to the appropriate clinical team
  • Track member engagement metrics and produce regular status reports for supervisors

What You Bring

  • 1+ years of experience in healthcare administration, care coordination, or medical office support
  • Familiarity with electronic health records (EHR) and care management software platforms
  • Exceptional organizational skills with the ability to manage multiple member cases simultaneously
  • Compassionate communication style with a commitment to member privacy and dignity
  • Knowledge of HIPAA requirements and healthcare confidentiality standards
  • High school diploma required; medical assistant certification or related coursework a plus
